When scheduling online, please list your "Reason for Visit" in the indicated field as well as any imaging (CT scan, X-ray, MRI) tests that have been performed, including when and where they were obtained.
The practice does not offer chronic opioid therapy. Our goal is to provide alternative effective approaches to pain management.
